The primary services we provide are:
NDIS support coordination
NDIS specialist support coordination
MACNI assessment, care plan development and care plan coordination
Secondary consultation services and coordination under other funding streams.
Experienced practitioners will work closely with you, your friends & family, your community and your other services to help you achieve your goals. Because everyone is different, we work with you in a personalised way, to make sure your needs and priorities are met.

NDIS Support Coordination
We are a registered provider of NDIS Support Coordination & Specialist Support Coordination for people living in Victoria.
We do more than just link you with the funded supports in your plan. We work with you to ensure you have the right supports around you to help you meet your needs and priorities.
DFFH & MACNI
Highly skilled and experienced practitioners provide comprehensive Care Plan Coordination, Assessments and Care Plan Development under the DHHS Multiple and Complex Needs Initiative (MACNI) to people living anywhere in Victoria.
Other Services
For organisations:
We also provide consultation to other organisations seeking to enhance their work with participants facing complex situations.
For individuals:
We offer support coordination services under other funding arrangements such as TAC. Please call us to discuss these options further.
